COMPLETING THE QUESTIONNAIRE

Your friend or relative will have been asked to complete a self-report version of this questionnaire [idQ]

Information from someone else (i.e. an informant) can be very useful which is why you have been asked to complete this version of the questionnaire [idQi]

This questionnaire covers the types of things that your friend or relative may like or dislike and the sorts of things that you feel he is good at, as well as some of those he may have difficulty with.

There are 100 statements in total and you will be asked to decide whether or not each statement describes him.

If the statement does describe him, you should click on the TRUE box. If the statement does not describe him then you should click on the FALSE box.

For example:

he-likes-football-true

However, if it does not describe him, click on the 'FALSE' box. This means that he doesn't like watching football on TV.


Sometimes the statement is expressed as a negative. For example:

he-doesnt-like-shopping-false

This response means that he does enjoy clothes shopping.

Those statements that are expressed negatively are sometimes more difficult.
It helps to say to yourself "Does this describe him?"

 

Please note that there may be some items where you could find it difficult to decide whether to click on the TRUE or FALSE box. You may wish that you had a SOMETIMES or DON'T KNOW box. This option is not available because of the specific way in which the questionnaire has been designed.

If there are any items that you find especially difficult, you can write notes about this in the COMMENTS box at the end. If you think that you may have a problem deciding how to respond, it would be helpful to be prepared (pen and paper handy or new Word document open) so that you can make notes on the items as you go along and then include these comments in the box at the end.

These comments can be especially useful if you are completing the questionnaire for someone as part of their diagnostic assessment.
